Build the record
Combine demographics, history, assessments, notes, medications, orders, diagnostics, and imaging.

EHR Simulation
ChartSlate is a free, privacy-first iPhone and iPad app for creating, exploring, and sharing simulated electronic health records.
Requires iOS & iPadOS 26.0+.
Faculty can build realistic patient charts and guided activities. Learners can search for cues, recognize trends, compare patients, identify safety concerns, and explain the reasoning behind their decisions, all without using real patient information.
User & Faculty Guide 2.2
Share the guide with learners, faculty, and simulation teams before they open ChartSlate. It covers reviewing charts, documenting learning, creating and importing cases, sharing course bundles, LMS use, and an Easy Start path for learners and faculty.
Free to view, download, post in an LMS, or share with your program.
More Than a Simulated EHR
Instead of simply reading a chart, learners use the information inside it to solve problems, support clinical decisions, and communicate their reasoning.
Built for Active Learning
Guide learners through structured activities that ask them to find evidence, recognize changes, identify errors, compare priorities, and connect chart findings to patient care.
Search the chart for the evidence that matters.
Create Realistic Simulated Records
Faculty can create simulated patient records with demographics, vital signs, laboratory results, medications, orders, assessments, notes, imaging, and other clinical information. Import table-based data, attach simulated materials, or begin with an existing case. Then add an activity that directs learners toward the decisions and evidence that matter most.

Bring the Chart into Simulation
During high-fidelity simulation, facilitators can release new orders, notes, vital signs, laboratory results, and assessments to nearby learner devices.
Learners receive evolving chart information as the patient’s condition changes, helping connect bedside assessment with documentation, clinical reasoning, and team communication.

Designed for the Whole Healthcare Team
ChartSlate supports multidisciplinary and interprofessional education across nursing, medicine, respiratory care, pharmacy, EMS, allied health, case management, and other healthcare disciplines.
The same simulated patient can be explored through different professional roles, helping learners understand both their own responsibilities and the contributions of the larger care team.
Connected to Bedside Coach
ChartSlate focuses on the patient record: finding information, interpreting evidence, recognizing trends, and building chart-based activities.
Bedside Coach focuses on the moment of care: handoff, assessment, intervention, communication, prioritization, escalation, reassessment, and reflection.
Compatible simulation case files allow educators to move selected case information between the applications, creating a connected experience from chart review to bedside action.
Portable case import and export require Bedside Coach Pro. Intended only for simulated educational cases, not clinical records or EHR integration.
Local-first by Design
ChartSlate does not require an account, use advertising, track users, or collect app data. Simulated records remain on the device unless the user intentionally shares them.
Cases and activities can be shared through supported Apple workflows such as AirDrop and Files, with nearby options for classroom and simulation experiences.
No account. No analytics. No tracking.ChartSlate 2.2
Free for iPhone and iPad
ChartSlate is free to use, with no subscriptions or in-app purchases. Requires iOS & iPadOS 26.0+. Create a simulated chart, build an activity, and bring active clinical learning into the classroom, skills lab, or simulation center.
